How to do I schedule an appointment?
Initial evaluations are arranged by direct physician referral or patient self-referral.  If an initial evaluation is requested by your health care provider, you will be contacted to set up your appointment.  If you have any previous relevant X-rays, MRI examinations, CT scans, EMG/NCV Nerve Conduction Studies, Operative Notes, or Examination Records, please bring them to our office for your first examination date. This will make the entire process flow more smoothly and not delay any possible diagnostic testing or treatments.  When an examination time is scheduled, a “check in time” will be given.  Please be sure to arrive on or before your “check in time” so that you will have adequate time to fill out all necessary paperwork and that you can be seen as promptly as possible.

Do you take my insurance?
We are currently contracted providers for the following health-insurance carriers: All lines of Anthem, Blue Cross Blue Shield, Federal Blue Cross Blue Shield, Harvard Pilgrim, Aetna, Cigna, Tufts, Tricare Tricare Prime, Martins Point, United Healthcare, Medicare, New Hampshire Medicaid, Workers Compensation and over 130 additional insurance carriers.  Please check with your benefits administrator or your insurance company regarding the specifics of your policy. It is your responsibility as the patient to obtain your referral from your primary care provider for evaluation and treatment in our practice prior to your visit if you have a HMO based insurance plan.

Do you treat children?
We treat infants, children, adolescents, adults and geriatric patients

What should I bring with me to my appointment?
Please bring your insurance information, a photo ID, any previous medical records, x-rays, MRI, CT scans, EMG/NCV studies, operative notes, consultations, IME evaluations and anything else that you feel would be relevant to your examination.  Consider bringing a book to read; appointments may be delayed due to emergencies and other circumstances.

What about workers compensation claims?
We participate with workers compensation carrier’s for injured workers.  Please provide a copy of the referral from your case manager, all pertinent workers compensation information including claim number and insurance company and any information regarding previous evaluations of your injury.  We welcome and highly advocate  that your case manager be present for all office examinations.  There is no additional charge for this service.

I need a prescription that Dr. Wilton wrote renewed, how do I do this?
Please call the office with your request during normal business hours (8:30-5:00 pm M-F) at least 48 hours before you need the medication.  Medications are not renewed after hours or on the weekends, except in extreme emergency situations.

I need some forms signed by Dr. Wilton, how does this happen?
Please submit the forms personally at our office approximately 72 hours before you need them to be completed.  Prepayment of a nominal fee is required for some forms.

Where does Dr. Wilton perform surgery?
Dr. Wilton is on the medical staff and performs lower extremity surgery at the following facilities: Stratham Ambulatory Surgical Center, Mercy Hospital-Portland Maine, Portsmouth Regional Hospital, York Hospital, Wentworth Douglas Hospital, Atlantic Plastic Surgical Center Portsmouth, Winchester Hospital-Winchester Massachusetts

If I have surgery, who performs the actual surgery?
Dr. Wilton performs the actual surgery, there are no students, interns, physician assistants, “physician extenders” or residents involved in your lower extremity care in the surgical center, hospital or office setting.  Dr. Wilton at his discretion will have other board certified surgeons who have advanced peripheral nerve surgical training and specialize in peripheral nerve surgery occasionally assist him with complex cases.  After your surgery, you will be followed by Dr. Wilton for all postoperative care.

Do you have a disclaimer?
Optimal outcomes not only depend upon arriving at a correct diagnosis, but also understanding each patient’s individual needs and expectations, and their unique circumstances.  Treatment should be completely individualized, and may vary from person to person.  Results from treatment may vary from person to person, and are not guaranteed.  Interventional surgical care is only offered if the risks outweigh the benefits of the proposed treatment.

What is your cancellation policy?
When you schedule your appointment at our practice, we reserve that time just for you. Please respect our time as we respect yours, broken appointments without calling to cancel are not accepted.  In the event that you need to cancel an appointment, please give at least 24 hours notice.
